Anya's portal didn't trigger when I completed the quest (in Hell). I can enter the halls of dead waypoint, but cannot trigger the portal. Is there anything I could do to fix this?

When you check the Quest Log, what does it say?

If you've killed Nihlathak, and already have the Halls of the Dead waypoint, you won't get Anya's portal.
