I have a hardcore Paladin (lvl 92) and a hardcore sorceress. I was doing mf runs on the sorc in andy NM for sojs and tried to join a new game when there was a message stating "Error when making game" or something to that effect. This was right inside the chat menu and not a failed to join game message. I figured I would log on and log back in, and when I did that I noticed my sorc has no avatar.. Additionally both join game and create game buttons are grey'd out, and I'm thinking, wait, I didn't die. And even if I did I would have a ghost avatar. I joined a chat channel, no avatar. However, when I'm not in a chat channel I have my sorc avatar. When logging into my account I have a blank avatar, nothing, just the name of my sorc.

I figured screw it, some kind of bug, I'll play my paladin for a bit. So I started doing mf runs. A few hours in, same bloody issue! I log out, log back in and my paladin has no avatar and create games and join games is greyed out. What's going on? This has been this way for 2 days now.

Well i can't tell you what has happend since i'm no Blizzard rep.
But i can tell 100% for sure that

- a hardcore character without avatar in character selections screen
- a hardcore character being invisible inside a chat channel
- a hardcore character with greyed out create/join games buttons

represents a dead hardcore character. That no avatar / insivible thing is a old bug that can be triggered by exiting the game with Alt-F4 like on an emergency situtation - which is not instant due to latencies - while your char still takes dmg OR your char has died and you left the game right after Death happend.

In any way: it's a dead character.
