The jist is after the events of D2 the cathedral attracted adventurers and treasure hunters, then they attracted innkeepers and merchants. A town quickly popped up outside of the ruins of the Tristram we once knew, then as the cathedral was emptied out the adventurers left and the town slowly died out.

The cathedral here was the site of the meteor strike you see in the announcement video, this is where the player starts.
