I imagine that little warrior or what ever.. he looks pretty stunned by what he is seeing..

As for Lilith, she left hell with Inarius because of "Love" or what ever it may have been..
She had her children (Us) I think she would want to protect them from everyone trying to kill them and be an ally..
Especially if Inarius is ideed the person in that concept art.. Id be willing to bet Lils will make an appearance

In the Sin War Trilogy she had no problem in murdering humans though... She also killed all the Angels and Demons in Sanctuary besides Inarius that proceeded to vanquish her to the void, from which she came back just before the Sin War events.

Also... from last time I think she would come back hating humans even more.
